If your child is as young as 12, you’ll find important information in Connections California. We want to help YOU create the smoothest transition to adulthood possible alongside your loved one!
Understand post-high school options! Learn how to become a self-advocate! Explore different housing options! Decide when to get started with the process! Become fluent in ALL of the adulting acronyms! Connect with other families also going through the transition to adulthood with their loved one!
Connections California: Transition to Adulthood is for any person with any disability. Everything is organized into five main categories. The goal is to help you find the information you need easily for this complex and exciting life transition!
Self-Determination Program Overview

Is your loved one a regional center client? Do you want to hear about a program that may bring them more freedom of choice and independence before and after their transition to adulthood?
The Self-Determination Program (SDP) provides individuals and their families with more control and responsibility in choosing services and supports to help them meet their life’s goals. The SDP is available to all eligible individuals receiving services from a regional center.
Please join an SDP representative from the Department of Developmental Services (DDS) as they:
Explain what the SDP provides participants
Introduce the 5 principles of self-determination: freedom, authority, support, responsibility, confirmation
Discuss how to begin your SDP journey
Describe the transition process from traditional services to the SDP
A representative from the Office of the SDP Ombudsperson will also participate in the discussion.
